Job Description
Segue Inc. Assertive Community Treatment (ACT) program is designed to engage persons with a Severe Mental Illness and with possible Co-Occurring Substance Use Disorder issues and whose needs are not met by traditional outpatient services. Services are delivered in the context and environment where they are needed (i.e. the team goes to the person).
The ACT Team Leader provides clinical leadership to the ACT Team in assessment, treatment planning, general healthcare, medical and psychosocial approaches.
Education Requirement
Master’s degree
Additional Requirements
Full or limited licensure to provide clinical supervision
Knowledge of the Mental Health Code, Medicaid Standards, and CARF standards are required
Knowledge of Standards of Best Practice for ACT and IDDT
Lives within a 45-minute radius of the local hospital to meet crisis response responsibilities
Salary is based on experience
